What is a DOT SAP?

A DOT Substance Abuse Professional (SAP) is a qualified clinician who evaluates a safety-sensitive transportation employee — such as a commercial truck driver — after that employee violates DOT drug and alcohol regulations, and manages their return-to-duty process.

Under Subpart O, after a driver’s drug/alcohol violation the SAP performs a face-to-face clinical evaluation, refers the employee to education and/or treatment, conducts a follow-up evaluation to confirm compliance, writes SAP reports, and sets a follow-up testing plan. A SAP is defined and regulated under 49 CFR Part 40 — the same clinician performs the evaluation, treatment referral, follow-up evaluation, SAP reports, and follow-up testing plan that let a driver legally return to safety-sensitive work.

What credential do you need to become a DOT SAP?

What knowledge does a SAP need?

Per §40.281(b), a SAP must have clinical knowledge and experience diagnosing and treating alcohol- and drug-related disorders, understand the SAP function relative to employer safety interests, and know 49 CFR Part 40, the applicable DOT agency rules, and the DOT SAP Guidelines.

What qualification training and exam are required?

Per §40.281(c), you must complete qualification training covering the required DOT SAP subject areas, then pass an examination administered by a nationally-recognized professional or training organization covering all elements of the training.

What continuing education must a SAP maintain?

Per §40.281(d), during each three-year period (from the date you passed the exam) you must complete at least 12 professional development hours (e.g., CEUs) relevant to SAP functions.

If you qualify as an addiction counselor rather than a physician, social worker, psychologist, EAP, or MFT, DOT accepts your certification only through one of these three national organizations — NAADAC, IC&RC, or NBCC.

Do you need a license to be a DOT SAP?

Yes — you must first hold one of six credentials listed in 49 CFR §40.281(a): a licensed physician (MD/DO), a licensed or certified social worker, psychologist, employee assistance professional (EAP), a state-licensed marriage and family therapist, or an addiction counselor certified by NAADAC, IC&RC, or NBCC. On top of that credential you must complete DOT SAP qualification training and pass a national exam.

Is SAP certification federal or state?

Both. The underlying professional license or certification is issued by your state (or by a national certifying board), while the SAP qualification itself is federal — the requirements are set in 49 CFR §40.281 and apply the same way in every state. The addiction-counselor path specifically requires certification through a DOT-accepted organization (NAADAC, IC&RC, or NBCC).

How long is a SAP qualification good for?

Your qualification stays current as long as you keep up your continuing education. Under 49 CFR §40.281(d), during each three-year period beginning on the date you passed your qualification exam you must complete at least 12 professional development hours relevant to SAP functions, and keep documentation showing you still meet every requirement.
